Preparing Young People for Life Beyond Care

What happens when a young person leaves residential or foster care and suddenly faces adulthood on their own?

OCTI’s Integrated Transition Program is designed to help caregivers, leaders, and organizations prepare young people for that critical transition—before the day comes.

Beginning around age 14, transition planning should become an intentional part of care. This practical training provides a four-phase approach to helping young people develop the skills, confidence, relationships, and plans they need for greater independence.

Participants will explore practical strategies for:

• Identifying strengths, interests, and future goals
• Building essential independent living and decision-making skills
• Preparing for education, employment, and vocational opportunities
• Developing healthy relationships and support networks
• Creating an individualized transition plan
• Moving from simply “aging out” to moving forward with purpose

Our responsibility is more than caring for children while they are with us. We must prepare them for the day they leave us.
