Attached is the information you need, including four ways to register. Please choose the option that works best for you.
If you send a photo or scan of your completed registration form from your phone to [email protected], please make sure it is easy to read.
Take the picture or scan in good lighting, remove as much background as possible, and make sure the form fills most of the image. place a sheet of white paper behind your form if the area behind is dark. Thank you for helping us keep registration forms clear and readable.
