Objectives >Bring about change in the health status of the community. >Identify orphans, children in distress and the terminally, chronically and temporarily ill >Provide pre and post counselling and ongoing support to the infected and affected. >Provide care and support to people infected and affected by HIV/AIDS. >Provide ad vocally services for the infeted and afected families. >Provide emergency food security to the terminally ill, the infected and the affected. >Offer capacity training to the elderly caring for the infected and affected families. >Establish support groups for the inffected and affected. >Liase with other agencies or departments dealing with orphans, HIV and AIDS matters. >Refer the infected and affected for further assistance >Organize education campaigns on issues pertaing to child abuse and HIV/AIDS >Decreas unemplyment by establing an income generating projects Beneficiaries >HIV/AIDS infected and aftected individuals. >The terminally ill and their children/orphans. >The youth, women, the elderly and people with different disabilitis. >Civil society >Stakeolders Clear Statementd of Project Deliverable/Inicators >Number of orphans identified, placed and monitored. >Number of vulnerable children identified and assisted. >Number of children and families benefiting from the programme. >Number of families with improved household food security. >Increase in the availability of care to the terminally ill, the infected and affected. >Increase in the number and type of resources available to immediate and urgent needs of children infected and affected. >Strategies/models development and implementations to protect children infected and affected by HIV/AIDS. >Number of youth receiving services and participation in HIV/AIDs programme. >Number of women receiving services and inform action. >number of identified teenage pregnancies within the families we serve Time Frame The project will continue as long as the need in the community exists. The funding applied for in this business plan is requested for a period of 12 months. The NGO will be constantly monitored to ensure the the money is utilized to benefit of beneficiaries. Resources Required For The project Human Resiurces >The project currently consist of 32 caregivers. The project co-ordinator will supervise the caregivers. >The co-ordinator will also be responsible to compile reports on beneficiaries and on services provided and manage the home Community Based Care Programme. Material resources >The organisation needs office equipment and stationary in order to executive their duties >The organisation also needs food parcels to provide families identified and the dropped in children and the older persons. Responsibilities Project Manager >Manage project >Fundraising and managing funds in collaboration with the treusure . >Supervision of caregivers >Develop work scheduless for caregivers. >To prepare reports (narrative and financial) to the service point. >Marketing the project. >Building relationships with relevant atakeholders and maintain them Service Point services Point Officer >Manage the project/programme. >Provide guidance and supervision to the project manage and the service provider >Regular monitoring and reporting to the provincial office. >Ensure placement of all referred qualifying children. >Provide debriefing services to the caregivers. >Create an enabling enviroment for service delivery for HIV/AIDS affected. >Facilitate capacity building for the service providers >Facilitate the establishment of child care forums. Local Aids Council >Assist in the monitoring of the projet. >advocate on the behalf of service provider Service Provider >Implement the business plan >Report on implemetation. Caregivers >Identification and referral of vulnerable children >Supervise families that receive food parcels >Encourage families to participate in programmes initiated for their benefit and for the benefit of their children >Facllitate the process of children to acquire registration documents and be placed in alternative care for those who qualify. >Aftercare programmes for children in distress-assisting in and supervising their school work >Empower children with skills >Provide basic counselling. >initiate neigbourhood support for child headed families. >Assist terminally, chronically and temporarily ill parents to develop memory boxes for their children. >visit the identified families three times in a week. >Provide care and support to terminally, chronically and temporally community members. Project Finance Budget >the cycle of payments will be within the finacial yeare 2012 - 2013 >The funding provided/allocated will be for food parcels for qualifying familise, school uniforms (provided the the need has been identified}, payments of stipends for caregivers and the project manager, training facilities, facilitating the support group for people living with HIV and AIDS and for office administration, especially stationary and equipments. Financial Control Measures >The funds will be transfered into two trenches of 50% each >The organizatio will be expected to support a financial report at the end of every month (i.e. before the fifth of every month) >expenditure shall be in line with the government/department financial prescipts (PFMA) >The finaciial report should state expenditures per funded item, enclose invoices and a bank statement for the month. >Failure to submit reports for three consecutive months will result in termination of the contract between the organization and the department. Sustainability The project will be linked with povery alleviation projects, donor funding projects and other govenment departments.
