A study can document distress, anxiety, or destabilization.
An institution must define what happens next.

Who monitors?
Who escalates?
Who follows up?
Who verifies continuity of care after the session ends?

As psychedelic access expands, the field will need more than safety language. It will need operational systems.

The question is no longer whether adverse events occur.

Federal psychedelic access is moving faster than post-experience infrastructure.

The April 18 Executive Order signals a new federal policy phase around investigational psychedelic drugs, Right to Try pathways, and ibogaine compounds under FDA review.

But access alone does not create duty of care.

The most underdeveloped part of the field remains the period after administration: the 48–72 hours when psychological openness, distress, confusion, or destabilization may emerge outside direct supervision.

The question is no longer only whether access should expand.

The question is whether institutions can build screening, monitoring, escalation, and continuity systems at the scale this policy moment requires.

The governance phase has begun.

Something shifted today. The White House signed an executive order directing the FDA and DEA to open pathways for psychedelic treatments, including ibogaine, under Right to Try. It mandates $50M toward state-federal partnerships and instructs the DOJ to begin rescheduling review for approved psychedelic drugs. We’ve been building safety infrastructure for this moment. The governance question was always when, not if. That timeline just moved.
