2026-27 SCHOOL IMMUNIZATION REQUIREMENTS
Does your child need updated immunizations or health screenings for the 2026–2027 school year? If so, now is the perfect time to schedule an appointment with your county health department or your child’s doctor!

Please do not wait until the end of summer. Health departments become extremely busy from late July through early August, and appointment times fill quickly before school begins. Scheduling early helps avoid the last-minute rush and ensures your child is ready for the first day of school.

County health departments can also provide the required completed forms needed for school enrollment:

• Form 3231 – Georgia Immunization Certificate
• Form 3300 – Vision, Hearing, Dental, and Nutrition Screening

Important Reminders:

📚 Children Birth–12th Grade
Students attending public or private schools or childcare facilities in Georgia must have Form 3231 on file showing required immunizations are up to date (or a notarized religious exemption). Students entering a Georgia public school for the first time must also have Form 3300 on file.

🎒 Rising 7th Graders
Students entering 7th grade (and new students in grades 8–12) are required to receive:
• Tdap vaccination
• Meningococcal (meningitis) vaccination
An updated Form 3231 must also be submitted to the school.

🎓 Rising 11th Graders
Students entering or transferring into 11th grade must receive a meningococcal booster vaccine unless the first dose was given on or after their 16th birthday. An updated Form 3231 is also required.

Registration is required for both RETURNING and NEW students. If your student is not registered, they will not be assigned to a class.
Before getting started, please remember:
• A current proof of residency is required and must be uploaded.

We encourage all families to complete registration as soon as possible to help us prepare for a strong start to the school year.

If you don’t have access to a computer or internet at home, no problem. Please reach out to your child’s school to ask about using a kiosk computer on campus to complete the registration.
