於
清朝丙子年 降駕馬鳴山.
民國丙子年 移駕五條港聚福宮.
民國七十七年歲次戊辰年 金身北上蘆洲濟世.

至今幾經遷徒
丙戌年陽月丁卯日建宮安座繼續服務各方信眾
