元始大天聖道院原起源於台南市六甲區龜港里，2013年奉神旨意興建新道院,於2014年農曆正月初一正式新建完成.

主要供奉元始大天王、元始天王、九天真王、太元聖母、九天道母、東華木公上相青童道君、西靈金母梵炁祖母元君、九天玄女、南海開古大聖祖、九天玄音娘娘、太乙救苦天尊、北天紫微真王及南天九陽尊王等眾神.
