本宮起源於民國46年間由已故劉勝東先生恭塑，在劉勝東先生70歲那年身體不適,由指示下擺設香案桌乞求上蒼後,身體日見康復於同年擇日開斧雕塑金身還願,於民國86年間吳府千歲指示前往祖廟台南麻豆代天府取火恭塑.金尊,於民國87年間指示擇日陞匾立宮名為{鳳邑混元玉虛宮};以上敘述之宮史由來
