主祀：天上聖母、大媽祖、二媽祖、三媽祖、四媽祖
同祀：千里眼將軍、順風耳將軍
配祀：保生大帝、吳府王爺、關聖帝君、中壇元帥
虎爺公祖、張府天師、天上王母娘娘、副駕四媽祖
慶典日期：每年農曆三月廿三日 天上聖母 暨 諸位神佛
合併祝壽 一仝慶祝聖壽無疆
本壇四媽祖奉玉旨於歲次乙巳年農曆十一月廿三日前往台南開基玉皇宮覲朝領旨 晉陞官職（二品天上四聖母）
