成立於西元1992年4月
歲次壬申年梅月
由宮主楊碧霞及眾老菩薩一同創立
主祀 觀音佛祖
配祀 準提佛母 地母至尊 王母娘娘
天上聖母 玄天上帝 關聖帝君
清水祖師 濟公師父 福德正神
中壇元帥小中壇元帥 虎爺將軍
偉陀菩薩 伽藍菩薩 五營將軍
宮主仙逝後因土地產權問題必須遷移由眾信徒發心成立管理委員會
搬遷於此
於西元2021年1月30日
歲次庚子年臘月十八日
進行入火安座大典
