民國37年，因本市信女李耍罹患重病四處求醫罔效，經前往朴子配天宮恭請天上聖母回家參拜而痊癒，一時傳遍坊間，日後更神威顯赫，信眾日增，上天旨意於民國53年成立天玄壇，嗣於70年晉升為天玄宮，農曆12月25日入火安座
