*IMPORTANT ANNOUNCEMENT TO ALL YOUTH WHO WANTS TO JOIN THE CAMP*

Step 1: Download the registration form on the QR Code below. If you are unable to download via QR code, you may click the link below to download the form:

Step 2: Fill up the registration & waiver form. Make sure that the signatures are originally signed (wet signature).

Step 3: Submit the forms to your church Youth Leader with the copy of the ID of your parent/legal guardian with three signatures (wet signature).

Step 4: Payment shall be collected by the Youth leader.

Step 5: Once the payment has been verified, you will be receiving an acknowledgement receipt and checklist for the things to bring.

PS: We will not accept forms which signed electronically.
