Manner of seminar: On-line or face-to-face
1. Name of proposed Coop or organization, if any;
2. Type of seminar;
3. Address or area where to Coop will be established;
4. Number of participants (not less than 15 persons)
5. Name of contact person;
6. E-mail address and phone number/s.

Manner of seminar/training : Face-to-Face
1. Name of the Cooperative;
2. Type of seminar/training;
3. Number of participants for each type of seminar/training;
4. Asset size - micro or small;
5. Name of the contact person;
6. E-mail address and phone number/s;
7. Official/registered address
* All seminars/trainings are on a first-come-first-serve basis;
* For on-line PRS : Zoom Link will be sent within 3 days prior to the scheduled seminar;
* For Face-to-face PRS : A Cooperative Development Specialist II (CDS II) will contact you to assist your group;
* For face-to-face FOC/GMC : Once your request is accepted a registration link will be sent 1 week before the seminar/training to be accomplished by each participant;
: Once submitted it will confirm your participation ( no registration no admittance);
: All participants are expected to be at the CDA NCR Extension Office at least 30 minutes before the start of the seminar/training.
