This form is for baccalaureate mass. Para po makafillup at maopen ang form magsignin po muna gamit ng gmail account. Any google account po ay pwede. Eto po ay para sa all levels. Nursery up to college graduates. Any awards/honor ay counted. Lahat ng grumaduate last year at ngayong year ay pwede magfillup ng form. Ang deadline po neto ay JULY 03, 2022. We will no longer entertain and close the form after July 03, 2022. Para po sa mga susunod na announcement ay ipopost na lang po namin. *formal picture/graduation pic/nakaschool uniform
*list of awards or honors
*complete name with course/strand/level of education
